The Social Outcomes Team designs and delivers programs to meet the needs, goals and aspirations of our diverse customers. Our programs work with individuals and groups, especially our youth, senior, and Aboriginal and Torres Strait Islander cohorts.

Customer Support Coordinators

Our Customer Support Coordinators provide coordinated support to Hume customers to promote wellbeing. Customer Support Coordinators are experienced in guiding our customers to access support and services in line with their goals and needs. For example, linking with mental health supports, NDIS, aged care providers, community groups and activities.

Community Programs

Our programs make a positive difference for our customers, their families, and community. Program Officers work with customers and other partner organisations to design and offer programs and activities in the following areas:

• Aboriginal and Torres Strait Islanders
• Over 55s
• Young people
• Economic development
